Service & care planning

Following a comprehensive assessment to identify a clients' health and social care needs, I will in partnership with the client and his/her family prepare a care plan tailored to their individual requirements, at all times guided by the clients' choices.

Services may include:

• Referral to community care/nursing organisations for ongoing social support and nursing care.
• Referrals for Allied Health assessments and ongoing care eg: Occupational Therapy,Physiotherapy,Podiatry.Diabetic Podiatry.
• Provision of nursing care direct to client.
• Provision of wound care and advise direct to patient.
• Referral to GP for assessment and team care arrangement.
• Liason with specialist eg: Vascular surgeon, Endocrinologist.
• Advise on accessing care packages (Consumer Directed Care).
• Overnight stay with client: eg: after cataract surgery.
• Weekend stay with client: eg to provide respite for carer / family.
